- How to Make a Report
  
  If you or someone you know has been a victim of sexual abuse of a minor in the past or present day by clergy (priest or deacon) you are requested to report the incident in the following manner:

  Department of Child Services: 800-800-5556
  Adult Protective Services: 800-992-6978
  Local Law Enforcement

  Thereafter, an investigation will be undertaken by or on behalf of the Diocese of Lafayette-in-Indiana.
  
  If you or someone you know has been a victim of sexual abuse of a minor in the past or present day by someone working in a parish or school for the Roman Catholic Church in the Diocese of Lafayette-in-Indiana (such as religious sister or brother, lay employee, lay volunteer, etc.) you are requested to report the incident in the following manner:
